We assume you filed Form W-4VT with the allowances its worksheet gives someone with one job, no dependents: 1 if you are single; 2 if you are head of household (yourself plus the head-of-household allowance); 2 if you are married filing jointly with one income in the household (you and your spouse). For the Child Care Contribution we assume your employer deducts the maximum allowed (0.11%).
